Siouxland Community Health Center has an opening for: RN Health Coach 1 – Full-time Successful candidate must have solid experience & advanced knowledge of: Minimum of RN license, BSN preferred. Current BLS and Mandatory Reporter certifications. Clinical office and case management experience preferred. Certified Diabetes Educator certification or ability to obtain within 6 months of employment. Experienced RN with a background in case management who is self motivated and capable of working with others to improve patient care. Successful candidate must be able to perform primary functions of position: Educate patients on chronic disease management and prevention Assist patients with identifying and setting proper self-management goals including coordination to determine an appropriate goal achievement Contact insurance companies for preauthorization/precertification approvals for doctor’s visits, procedures, diagnostic test, etc. Including maintaining detailed records in the EMR of all preauthorization activities.
